Key Responsibilities:- Conduct Care Act assessments and reviews
- Develop and implement personalised care and support plans
- Carry out safeguarding investigations under Section 42
- Work with adults with learning disabilities, mental health needs, physical disabilities, or older adults
- Liaise with families, carers, and multidisciplinary teams
- Ensure compliance with legal and ethical frameworks
- Qualified Social Worker (Degree or equivalent)
- Registration with Social Work England
- Minimum 1 year of post-qualifying experience in Adult Services
