This is a fantastic opportunity for experienced clinicians looking to make a real impact in urgent care and crisis intervention in the community. About the Role: As an Urgent Community Response Practitioner, you will: Respond rapidly to patients in crisis within the community to avoid unnecessary hospital admissions. Work autonomously and within a multidisciplinary team to assess, treat, and support patients in their homes or care settings. Deliver high-quality, patient-centred care with a focus on recovery, safety, and independence. Liaise with GPs, social care, community teams, and acute services to ensure continuity of care. Who We’re Looking For: Qualified Registered Nurse, Paramedic, or Allied Health Professional. Experience working in urgent care, community health, crisis response, or A&E. Able to work independently and manage caseloads under pressure. Excellent clinical decision-making and communication skills.
