Service Care Solutions are currently recruiting for Urgent Community Response Practitioners to join a busy and supportive NHS team on a full-time agency basis. About the Role:

  • Respond rapidly to patients in crisis within the community to avoid unnecessary hospital admissions.

  • Liaise with GPs, social care, community teams, and acute services to ensure continuity of care.
