Permanent Full-time - 35 hours per week

The role:

Join a dynamic and supportive therapy led team and embark on a fulfilling journey as a Community Reablement Assistant. In this flexible and rewarding role, you will be working alongside Occupational Therapists and Reablement Officers playing a vital part in helping individuals regain their confidence and independence after a hospital stay. As the first point of contact for those returning home, you’ll provide essential support and bring positivity to their recovery journey, making a meaningful difference in their lives every day.

Your main responsibilities:

  • Supporting people with their personal care and empowering them to do as much as possible on their own.
  • Monitoring and providing feedback on the progress of people’s goals.
  • Assisting with medication and encouraging independence.
  • Completing care records and other relevant paperwork.
  • Facilitating access to community groups and services.

Shift times available:

  • 07:00-14:30
  • 09:00-14:00
  • 15:00-22:30
  • 17:00-22:00

No prior care experience? No problem! They’ll provide all the training and support you need to succeed and grow in your role. A valid licence to drive in the UK is required as the role involves travelling locally and lone working.
