We’re looking for passionate, proactive Complex Care Coordinator to join an established and highly accredited Healthcare provider in the UK. This role involves monitoring and reviewing care plans, coordinating nurses and care workers, and ensuring efficient allocation of care visits. You will be responsible for the seamless operation of a wide geographical area, ensuring high standards of care and supervision.

Essential: You will require Level 2 in Health and Social Care or above / or Proven competence in the role and willing to undertake qualifications.

Key Responsibilities:
- Coordinate nurses and care workers to deliver top-quality care.
- Address and resolve day-to-day issues raised by service users.
- Handle enquiries from nurses and carers, escalating when necessary.
- Use Careplanner to fill unassigned shifts promptly and effectively.
- Ensure no shift goes unfilled, possibly attending shifts at short notice.
- Review and update care plans and risk assessments regularly.
- Conduct staff interviews, matching skills and experience with service user needs.
- Monitor service quality and report to management to maintain high standards.
- Report changes in service users' health and support needs.
- Identify and address potential abuse situations, reporting to the Registered Manager.
- Collaborate with other care agencies and service providers.
- Promote the organisation positively and identify business development opportunities.
- Undertake tasks as directed by the Registered Manager.
