We are looking for an Adult's Social Worker to join a Mental Health Hospital Discharge Team. This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.
About the team: This is a fast paced team who work to ensure patients with mental health conditions are safely discharged from hospital and back into community settings. Carrying out bedside assessments (such as Section 117), arranging appropriate care packages for comprehensive discharges and advocating for service users and their independence is at the forefront of the team's motivation. The team work with professionals in the hospital, community resources teams and the voluntary sector.
About you: The successful candidate will have worked in a mental health specialised team (inpatient or community based) with vast knowledge and experience of completing Section 117 assessments. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years of experience working in an Adult's Social Work environment is essential in order to be considered for this position.
