Locum Community Occupational Therapist – Discharge to Assess (D2A) Pathway Location: Basildon, Broomfield, Southend (Mid and South Essex) Salary: £25 – £27 per hour Job Type: Full-Time or Part-Time, Ongoing Contract Start Date: ASAP Job Description: We are currently recruiting for two Occupational Therapists to join a dynamic and supportive team delivering care across the Discharge to Assess (D2A) pathways in the Basildon, Broomfield, and Southend areas. This is an excellent opportunity to make a meaningful impact by helping patients transition safely from hospital to home or community settings. Key Responsibilities: * Undertake holistic assessments and interventions in community and care home settings * Identify and address therapy and care needs to reduce hospital re-admissions and promote independence * Develop, implement, and evaluate person-centred care and therapy plans * Provide timely equipment assessments and support discharge planning * Set rehabilitation goals to enhance function and quality of life * Work autonomously within professional and organisational guidelines * Coordinate with multi-disciplinary teams to ensure the most appropriate pathway for each patient Requirements: * HCPC-registered Occupational Therapist * Valid UK driving licence and access to own vehicle with appropriate business insurance * Willingness to travel across Mid and South Essex (area-specific roles may be negotiable) * Community experience or previous work within D2A pathways desirable * Commitment to person-centred, safe, and effective care What We Offer: * Competitive hourly rate: £25 – £27 per hour * Flexible working options – full or part time considered * Ongoing contract with opportunity for extension * Supportive team and access to appropriate training for competency development
