Rate: £28 an hour

Overview

Eden Brown are seeking an enthusiastic qualified social worker to join Birmingham City Council within the Transitions Team, working with young people aged 18-25.

Responsibilities

1. Conduct conversations and support planning for young adults.
2. Manage commissioning of care forms and budget requests.
3. Perform reviews, mental capacity assessments, and attend relevant meetings such as EHCP and Section 117 reviews.
4. Complete CHC checklists as part of case management.
5. Work collaboratively with Senior Practitioners and the Team Manager.
6. This role offers a hybrid working basis.

Requirements

• Qualified Social Worker
• Minimum of 2 years post-qualification experience
• Enhanced DBS certificate
• Experience in a transition team is advantageous
