Are you an experienced Complex Care Nurse looking for a new challenge in complex care? Are you looking to develop your leadership skills? Join a well-established provider of complex care packages as a Nurse Case Manager! This role will be based in the South London catchment area where the company has secured further packages during an exciting growth phase.

The successful Nurse Case Manager will be responsible for the overall mobilisation of complex care packages, mainly based at head office but occasionally going out into the community to complete assessments, managing the transition from hospital to community setting, creating personalised care plans for each service user then handing over to the allocated nurse manager. You will work closely with a core team of case managers and support them with the onboarding of new packages.

To work as a Nurse Case Manager, you must:

  • Be an RGN with a minimum of 2 years experience in community complex care
  • Possess a supportive management style that can bring out the best in others
  • Have up to date experience of tracheostomy care and ventilators
  • Have advanced communication skills verbally and written to liaise effectively with patients, families and the MDT
  • A full UK Driving License desirable but not essential

The key responsibilities include:

  • Designing and delivering a tailored care plan specific to an individual client's needs
  • Effectively managing the MDT through strong communication and teamwork
  • Providing ongoing monitoring, support and assessment of clients' physical and physiological needs
  • Building and maintaining sophisticated relationships with the ICBs and hospitals
  • Working closely with community teams, CHC nurses and CCG commissioners to establish and maintain bespoke packages of care
