Transition Care Case Manager Allied Health (Grade 2) or Nursing
24 hours per week – Part Time, Ongoing St Georges Health Service

About the Role
An attractive position is available for an experienced allied health or nursing professional to join our Transition Care team working as a case manager based at St George’s Health Service in Kew.
The Transition Care case manager plays a key role in coordinating and case managing clients within the Transition Care Program as part of a multidisciplinary team.
Duties
- Provide high quality, evidence-based, patient-centered care.
- Undertake holistic assessments in collaboration with clients, using case management principles and frameworks including strengths-based approaches.
- Collaborate with clients, carers, and families to develop care plans considering health, functional, psychosocial, cultural, environmental, and economic needs.
- Facilitate client-generated goals that are realistic, time-limited, and aimed at improving health and wellbeing.
Classification Range
The classification for this position is:
- Allied Health Grade 2 Year 1-4 ($88,467 – $101,691 per annum, pro rata)
- or Registered Nurse Grade 4A Year 1-2 Community Health (YW7 – YW8) ($107,692 – 113,360 per annum, pro rata).
Your Contribution
- Tertiary qualification in Allied Health or Nursing.
- Minimum of 5 years’ experience in healthcare.
- Ability to work autonomously and in a team.
- Experience working with service providers to achieve client outcomes.
- Excellent communication, negotiation, organization, planning, and problem-solving skills.

About Transition Care Program
This program supports older individuals post-hospitalization to complete their recovery, optimize function, and access long-term care. The team includes various health professionals and administrative staff.
