Patient Care & Treatment: Provide comprehensive nursing care to housebound patients, ensuring dignity and respect at all times. Implement and review care plans tailored to individual patient needs. Administer oral, IV, and intra-muscular medications while monitoring patient reactions. Perform wound management, including compression bandaging and diabetic foot checks. Manage PICC and Hickman lines, including ongoing care and maintenance. Conduct phlebotomy, cannulation, and immunisation, including seasonal flu vaccinations. Perform catheterisation (male, female & suprapubic) and removal of stitches and clips.
Preventative & Hospital Avoidance Care: Participate in hospital prevention schemes, providing home-based treatment for conditions such as COPD to reduce hospital admissions. Collaborate with multidisciplinary teams, including GPs and social care professionals, to ensure coordinated patient care. Advocate for patients by ensuring their health and social care needs are met within the community setting.
Safety & Compliance: Adhere to strict medication safety protocols, ensuring correct dosages before administration. Maintain accurate patient records and documentation in line with NHS guidelines.
