This is a full-time, 40-hour per week position, working either 7a-7p, or 7p-7a, Wednesday-Sunday, offering stability and the opportunity to make a meaningful impact on the lives of seniors in our community.

Position Overview
As a Care Partner Lead, you will serve as a reliable on-call caregiver in the event of call-offs. You will be on standby during your designated schedule (Monday-Friday or Wednesday-Sunday) and will be contacted when coverage is needed. Your role is vital in ensuring seamless care for our clients. This is a leadership position and will require your attendance and support during Pre-Employment training of new Care Partners. You will serve as a mentor and ongoing trainer to our Care Partner staff. This includes introducing new Care Partners to their clients, allowing Care Partners to shadow you while providing services at a client’s home, and following up with Care Partners as it relates to quality assurance. Additionally, Touching Hearts at Home may occasionally require support with administrative tasks and community events. As a Care Partner Lead, you will be expected to assist with these activities as needed, following requests from the Touching Hearts at Home Scheduler or Executive Director. Care Partner Leads receive cross-training in our scheduling process and software and are expected to assist with scheduling as needed. This may involve working in the office during regular business hours or serving as an on-call representative after hours. Whether you are filling in for a call-off or assisting with other tasks, you are guaranteed 80 hours of pay per bi-weekly payroll cycle. If you exceed 40 working hours in a single week, you will receive overtime pay at the applicable rate, in accordance with state and federal labor laws. If a Care Partner Lead declines to fill in for a call-off or fails to report for duty as requested by the Scheduler or Executive Director, the guaranteed hours for that week will be forfeited, and only the actual hours worked will be paid for that payroll cycle. Additionally, repeated refusals may result in disciplinary action, up to and including termination of employment.

What We’re Looking For
Professional Experience: Professional experience as a Home Health Aide (HHA), Personal Care Assistant (PCA), Certified Nursing Assistant (CNA), Licensed Practical Nurse (LPN), Registered Nurse (RN), or Companion Caregiver
Valid Driver’s License & Insured Vehicle (for client transportation as needed)
Reliable Cell Phone & Internet Access (for communication and scheduling)
Availability: 24/7 standby Monday-Friday or Wednesday-Sunday
Willingness to Travel: Up to 40 miles for shifts
Preferred Qualifications: Certification in HHA, PCA, CNA, or LPN
Physical Requirements: Ability to lift, push, and pull up to 25 lbs
Must pass background check, professional reference check, and driving record verification
