Responsibilities:
- Deliver high-quality care to improve patient outcomes in line with Diabetes guidelines
- Identify and optimise care for patients with sub-optimally controlled Type 2 diabetes
- Conduct point of care testing and refer for specialist insulin treatment as needed
- Provide education, support, and advice to patients with Type 2 Diabetes
- Fulfil corporate nursing responsibilities and contribute to a proactive care approach
Requirements:
- A Diabetes diploma or equivalent EN qualification
- At least 3 years of post-registration experience managing diabetes in primary care
- Strong knowledge and confidence in providing education and advice to Type 2 Diabetes patients
- Skills in optimising oral hypoglycaemics
- A full driving licence and current NMC registration
- IT literacy, including primary care systems
