About the Role
- Conduct needs-led assessments in line with the Community Care Act.
- Develop personalised care plans and coordinate multi-service care packages.
- Work collaboratively with service users, carers, and multi-disciplinary teams to ensure seamless service delivery.
- Manage complex caseloads, including safeguarding adults and Deprivation of Liberty Safeguards (DoLS).
- Support the team in achieving performance targets and driving continuous improvement.
- Ensure all work aligns with Brent Council's policies and procedures.
What We're Looking For
- A qualified Social Worker with current Social Work England registration.
- Experience in adult social care.
- Strong Front Door or case management skills, with the ability to prioritise effectively.
- Excellent communication and teamwork skills to collaborate across services.
