About the Role

You'll be joining a proactive and supportive Access and Wellbeing Team focused on prevention, enablement, and strength-based support. This includes medium to longer-term social work interventions, safeguarding enquiries, reassessments, and reviews.

Key Requirements

  • Must be available to be physically present in the office 3 days per week

Role includes:

  • Home visits
  • Joint working with other professionals
  • Duty responsibilities

About You:

  • Qualified and progressed Social Worker, registered with Social Work England.
  • Completing Care Act assessments
  • Court of Protection and Human Rights assessments
  • Undertaking Carers Assessments
