We seek warm, professional Outreach Workers to work in Rochdale at Personalisation service. Successful candidates will provide flexible, personalised support to people with support needs (mental health needs/learning disability/physical health needs), to enable them to develop the skills to lead independent lives, promote their quality of life and personal development.
You will engage with service users at our Rochdale service, building trusting therapeutic relationships, working to encourage a positive life outlook and coping skills. Your role as Re-enablement Outreach Worker will include visiting service users in their own homes or in other settings as part of a structured approach. You will work closely with other professionals and agencies to provide a co-ordinated service which meets the identified needs of the individuals.
Our Rochdale service operates 7 days a week, 365 days a year and you will be expected to work on a rota system undertaking shift work as required. Evening and weekend work is expected.
Full training is provided, as is the opportunity to work towards QCF Diploma in Health and Social Care. Benefits of working with Creative Support include a probationary bonus, pension contributions, 28 days Annual Leave and company paid enhanced DBS.
