Are you an experienced Registered Mental Health Nurse (CPN) ready to make a difference in your community? Service Care Solutions is seeking a compassionate, confident and committed Band 6 Community Psychiatric Nurse to join the Scarborough Community Mental Health Team (CMHT).
Your Role: As a vital member of the multidisciplinary team, you will:
- Provide comprehensive mental health support to service users in the community.
- Conduct initial assessments of new referrals and manage a caseload of complex clients.
- Act as a care coordinator or lead professional, ensuring high standards of care planning and risk management.
- Deliver clinical advice and support to colleagues and partner services.
- Collaborate closely with local authority and social care teams to deliver person-centred, holistic care.
Requirements: Registered Mental Health Nurse with a valid NMC PIN. Community Mental Health experience essential.
