About the Role

Atlantis Medical is seeking a Band 6 Occupational Therapist to join the Community Falls Service at Lewisham and Greenwich NHS Trust. This role focuses on preventing falls and promoting independence among older adults by assessing risks, prescribing and fitting equipment, and implementing tailored intervention plans.

As part of a multidisciplinary team, you will work closely with patients in their homes and care facilities, helping them maintain mobility and safety. A car owner/driver is essential for this role, as travel between locations is required.

Key Responsibilities

  • Assess fall risks and develop treatment plans.
  • Prescribe and fit mobility and adaptive equipment.
  • Educate patients and caregivers on fall prevention.
  • Work with a multidisciplinary team to enhance patient safety.
  • Maintain accurate patient records and adjust care plans as needed.

Requirements

  • HCPC registration as an Occupational Therapist.
  • Experience with older adults in a community setting.
  • Knowledge of falls risks and interventions.
  • Car owner/driver (essential) for travel between sites.
