Job Overview: We are seeking a Band 6 Occupational Therapist to join the Stroke and Neuro Rehabilitation team at Alexandra Park Health Centre. This role requires prior experience working at Band 5 level and a strong background in stroke and neurological rehabilitation. A driving licence is essential due to community visits.
Key Responsibilities:
- Deliver high-quality occupational therapy interventions for patients recovering from stroke and neurological conditions
- Carry out comprehensive assessments and develop personalised rehabilitation plans
- Support patients in achieving functional goals to improve independence and quality of life
- Work collaboratively with the multidisciplinary team to ensure coordinated care
- Participate in discharge planning and community-based follow-ups where required
