The Substance Misuse Service (SMS) provides clinical interventions for prisoners with 'Substance Misuse' health needs at HMP Aylesbury. The post holder will work as a member of the SMS team to deliver high-quality, safe, and effective Non-Medical Prescribing Services. The post holder will be responsible for delivering and monitoring high standards of substance misuse nursing care to patients assessed and admitted to the ISMS, in accordance with their clinical needs. This involves applying evidence-based practices to achieve positive clinical outcomes and adhering to relevant clinical protocols and policies. The post holder must be thoroughly familiar with all aspects of security and safety within a custodial setting.

Key responsibilities include:

  • Assessment, planning, implementation, and evaluation of care plans for patients referred to this specialist service.
  • Providing advice on treatment options, including substitute prescribing, management of alcohol withdrawals, detoxification, titration, and symptomatic relief.
  • Dispensing medication, including controlled drugs, and managing all aspects of clinical safety, such as overdose prevention, in a high-risk environment.
  • Participating and leading multidisciplinary team meetings and case discussions.
  • Attending SMS team meetings, 'Medicines Management' group, and other delegated meetings.
  • Ensuring regular handover meetings and effective communication across the team, utilizing electronic record systems for auditable communication within the prison and with other healthcare services.
  • Providing specialist advice and support to colleagues throughout the prison.
