An Outreach Support Worker - mental health to provide practical and emotional support to individuals, often in their own homes or community, to promote independence, recovery, and access to resources, while also advocating for their rights and needs.

Key Responsibilities:
- Providing support in the individual's home, such as assisting with daily routines, attending appointments, or accessing services.
- Dealing with caseloads.
- Facilitating social activities or assisting with accessing community resources.
- Supporting individuals in accessing benefits, housing, or other services.
- Providing immediate support during a crisis, such as de-escalation techniques or accessing emergency services.
- Conduct needs assessments to identify individual goals and develop personalized support plans.
- Offer emotional support and encouragement to individuals experiencing mental health challenges.
- Respond appropriately to crises and challenging behaviours, ensuring the safety and well-being of individuals and staff.
