Join Our Hospital Discharge Team – Social Care Assessor Opportunity
Are you ready to make a meaningful difference in the lives of patients as they transition from hospital to home? We’re seeking a Social Care Assessor to join our Hospital Discharge Team on an initial 3-month contract, with the possibility of extension. This part-time position is for 17.5 hours per week.
