About you

You’ll be a qualified social worker or occupational therapist with a minimum of 2 years post qualifying experience. In addition, you will also need the following:

  • Actively undertakes a specialist role i.e. Moving and Handling Practitioner, Practice Educator, Approved Mental Health Practitioner, Best Interest Assessor or agreement to undertake a specialist role within the first year of the role.

You will have extensive knowledge and experience in the field of adult social care, with an emphasis on safeguarding practice and related legislation. You will have the ability to manage competing service priorities and a sound grasp of current legislation, personalisation agenda and meeting the business objectives of the department. Demonstrating management potential, you will have excellent all-round communication and negotiation skills working with the Statutory, Voluntary and Independent sector partners.

You will adhere to Hertfordshire’s Connected Lives Practice Principles by ensuring that people are supported to maintain their independence and receive person-centred care and support. You will utilise your knowledge of the Care Act 2014, Mental Capacity Act 2005, and other social care legislation, to manage the most complex cases. You will use your advanced knowledge and experience to support other members of the team and the management team to ensure good practice. You will develop close working relationships with hospital staff and care providers to ensure effective care planning and balance the use of resources.

About the role

As an Advanced Practitioner, you will lead on peer development and practice, support and supervise staff to develop their skills and knowledge in line with related legislation including The Care Act and Hertfordshire’s Connected Lives principles to contribute to the ongoing development of the service.

Advanced Practitioners are responsible for delivering high quality statutory social care including recognising safeguarding, delivering assessment, supervision and care management functions as well as developing innovative, creative community solutions. Advanced Practitioners are pivotal in developing links with the local community through undertaking multi-agency work with partner organisations, providers, professionals, individuals, families, and carers.

There is responsibility for ensuring that all statutory health and social care responsibilities are met and delivered to a high standard including safeguarding, assessment, supervision, and care management functions. We are re-energising our adult social care model of Connected Lives Approach that has a strong ethos of strength-based working, identifying personal goals and ensuring personal, family and community resources are utilised to achieve positive outcomes for people.
