About the role
An exciting opportunity has arisen for five new roles across our Prevention and Early Intervention Assessment Team, as well as our Transfer of Care Team. Our Transfer of Care Team is dedicated to managing patient discharges from acute and community hospitals across Cambridgeshire. We pride ourselves on a Home First approach, ensuring individuals can stay within their own communities while receiving the care and support they need.
The Prevention and Early Intervention Assessment Team provide support to those adults who have had a period of Reablement or Intermediate Care team support. We also provide Carers Assessments to those in informal caring roles whose cared-for person is not in receipt of statutory services. Our teams are multidisciplinary and offer support to individuals and also to families and carers. The team work closely to support and share information on prevention services across the county. We have multiple bases in the county and can agree on a contractual base when you are appointed to the role.
You won’t need to have a social work qualification but knowledge, experience and understanding of the needs of vulnerable adults will be essential to succeed in the role. Working knowledge and experience of the Care Act, Mental Capacity Act and Cambridgeshire County Council's Adult Social Care policies and procedures will also be an advantage, particularly around wellbeing and outcomes.
Interviews are planned to take place on the 07th and the 08th of April. The locations are Countywide, Hinchingbrooke Hospital, Long Road, Huntingdon PE29 6NT and Addenbrookes Hospital, Hills Road, Cambridge, CB2 0QQ. About you
You will need to have good and accurate literacy, numeracy and administrative skills and the ability to analyse information to solve general queries. It is imperative that you have strong communication skills, a confident telephone manner and can work on your own initiative. Good planning and organisation skills will be key, and you should be used to working under pressure to meet deadlines, allowing flexibility and resilience. You will need a good working knowledge of the Care Act 2014 and Mental Capacity Act 2005. Experience of multi-disciplinary working to enable best outcomes for individuals.
