We are looking for an enthusiastic, proactive nurse to work within our Community Nursing Team. You will be working autonomously, across the borough, to provide holistic, child and family centered care for children and young people with SEND, PMLD and complex health needs, ranging from 2 - 19 years of age.
This is a dynamic role encompassing:
- Assessment, planning, delivery and evaluation of clinical nursing care;
- Writing appropriate care plans to ensure safe and effective care delivery;
- Delivery of specialist nursing care, including but not limited to; gastrostomy management, wound care, suction;
- Delivery of specialist training for families and school support staff;
- Multi-disciplinary liaison to ensure continuity of care and attendance at multi-professional meetings, including child protection case conferences;
- Provision of support to families, including signposting to specialist services and completing onward referrals for care;
- Providing health advice for education, health and care plans.
