We are seeking a compassionate and experienced Health Coach to join our team. The ideal candidate will have a strong background in health coaching, with a focus on supporting chronic health patients. You will work directly with patients to develop personalised care plans, provide emotional support, and help them navigate their cancer journey. Additionally, you will deliver one-on-one and group sessions to support patients' health and fitness goals.
Key Responsibilities:
- Conduct one-on-one and group coaching sessions with cancer and chronic health patients to assess their needs and develop personalised care plans.
- Provide ongoing support and motivation to patients, helping them achieve their health and wellbeing goals.
- Collaborate with healthcare professionals to ensure a holistic approach to patient care.
- Educate patients on healthy lifestyle choices, nutrition, and stress management techniques.
- Monitor patient progress and adjust health plans as needed.
- Maintain accurate and confidential patient records.
Qualifications:
- Certified Health Coach and/or CanRehab Personal Trainer L4 qualification, or a degree in Sport Science.
- Experience working in a healthcare setting is preferred.
- Strong understanding of cancer treatment and its impact on patients' physical and emotional well-being.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
