We are seeking a qualified Social Care & Health Professional to join our team and contribute to the well-being of individuals within our community. The role involves providing high-quality social care and health support to vulnerable adults or children with a variety of needs. You will be responsible for delivering person-centred care, working in collaboration with health professionals, and ensuring compliance with relevant health and social care legislation., Deliver person-centred care plans to individuals, ensuring that all care needs are met with dignity and respect.

Assess the social, physical, and emotional needs of service users and develop, implement, and review care plans accordingly.

Provide direct personal care, including assistance with daily living tasks, mobility, medication management, and personal hygiene, as required.

Advocate on behalf of service users and support them in making informed decisions regarding their care and support options.

Work with healthcare professionals (e.g., doctors, nurses, therapists) to ensure seamless delivery of services and continuity of care.

Assessment and Evaluation:

Conduct thorough assessments of individuals’ needs, abilities, and risks to determine the appropriate level of care and support.

Regularly review and update care plans, ensuring they remain responsive to any changes in the service user’s condition or circumstances.

Maintain accurate records of assessments, interventions, and outcomes in compliance with regulatory standards.

Safeguarding and Compliance:

Ensure compliance with relevant legislation, policies, and procedures (e.g., Care Act 2014, Children Act 1989, Safeguarding Vulnerable Adults & Children).

Identify and respond to safeguarding concerns, following safeguarding procedures and ensuring the protection of vulnerable individuals.

Maintain confidentiality and respect the privacy of service users at all times.

Collaboration and Communication:

Work effectively within a multidisciplinary team, including social workers, healthcare providers, and family members.

Participate in case meetings, care reviews, and team discussions to ensure the best possible outcomes for service users.

Provide support and guidance to less experienced colleagues and trainees as required.

Administration and Reporting:

Keep up-to-date records of care provided and other relevant information using appropriate software and systems.

Produce and submit reports, assessments, and care reviews in a timely manner.

Relevant social care or health qualification (e.g., NVQ Level 3 or 4 in Health and Social Care, Social Work Degree, Nursing Degree, or equivalent).

Demonstrable experience in the social care or healthcare sector, working with vulnerable individuals.

Strong knowledge of current social care and health policies, practices, and legislation in the UK.

Good understanding of safeguarding procedures and protocols.

Excellent communication, interpersonal, and organisational skills.

Ability to work independently and as part of a team.

Empathy, patience, and a strong commitment to providing high-quality care.

Full DBS (Disclosure and Barring Service) check and a valid driving licence (depending on the role and location).

Desirable:

Additional professional qualifications in a specialist area of social care (e.g., learning disabilities, dementia care).

Experience in case management or supervising staff.

Knowledge of care management software and record-keeping systems.
