As our Support Coordinator, you'll be at the heart of supporting our customers who have complex needs to develop their skills to live more independently in the community. You'll make a big difference each day as you help our customers achieve their hopes and aspirations.
Typical day as our Support Coordinator:
- Meeting with customers to create support plans and determine their support needs.
- Supporting customers with daily living activities such as preparing meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation.
- Carrying out risk assessments, support planning, goal setting and regular reviews.
- Facilitating property maintenance and Health and Safety checks.

You bring:
- Passion to support our customers to live their best life, working collaboratively with an eye for detail.
- Experience of creating person-centred support plans and working as a team to accomplish customers' goals.
- Experience of coordinating and assessing customer referrals.
- The ability to work on your own initiative, remain calm under pressure and have a resilient approach.

Job details:
We are as flexible as we can be to support our colleagues. Where we're flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion. We work on a rota basis, Monday to Sunday between the hours of 7am & 7pm. This includes Bank Holidays and Christmas.
You must be able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues. You'll need an Enhanced DBS check done and we pay for that.
