Band 6 Care Co-ordinator (Occupational Therapist, Social Worker, Nurse)
- To be responsible for the co-ordination, commissioning and personal delivery of individual packages of care for a caseload of service users within CMHTs.
- To undertake initial assessments of new referrals to the service.
You must be HCPC registered to be eligible for the role.
