Salary: £12.21 per hour

Hours: 15 hours per week

Contract length: Fixed-term until 31 March 2026

Location: Community - mainly Bridgend & Rhondda Cynon Taf with some travel throughout Cwm Taf Morgannwg region

Reports to: Home First Programme Manager

Main Duties and Responsibilities:

  • Work under the direction of the Home First Programme Manager to provide practical and emotional support to older people to enable them to remain living independently in their own home for as long as possible.
  • Support older people in their own home, Community, and other settings, to regain and retain their independence and feel connected to their community.
  • Reassure, motivate and encourage older people to maintain quality of life.
  • Support the safe discharge of a service user to their homes from a hospital stay.

Responsibilities:

  • Ensure the rights, dignity, welfare, confidentiality, and privacy of older people.
  • Provide a hospital transport service for patients leaving hospital, including taking home medication and equipment.
  • Settling them in their own home, i.e., checking cupboards for food, ensuring the heating is working and on etc.
  • Provide Carer support when visiting the service user at home, by offering practical assistance, i.e., prepare a drink, lunch, check provisions etc.
  • Providing person-centred support to older people in the community.
  • Assist with maximising income, including benefit claims and signposting to relevant services.
  • Liaising with relevant service providers as necessary, including meals on wheels, housing, care and repair, social services, etc.
  • Following MECC (Making Every Contact Count) principles in all contacts with clients.
  • The job holder will be expected to be a lone worker aiding older people in the home, community and other settings.
  • Maintaining up to date digital case records and supporting the digital inclusion and independence of older people.
  • Attend mandatory training, one to ones, team meetings and staff away days and acting within the policies and procedures of the organisation.
  • Flexibility will be required as determined by the needs of the service.
  • Promote Age Connect Morgannwg services.
  • Any other duties relevant to the post as directed by the Home First Programme Manager.
  • Work in accordance with policy and procedures relevant to the post.
