Islington Learning Disability Partnership - Adults and Transitions - Social Work with a minimum of 2 years experience.
There are currently two different roles across the service being recruited for: 2x vacancies. One of the roles is working to complete the backlogs of reviews/assessments across the service, mainly in the LD adults side of the service. There is a strong emphasis on someone who is capable and confident in completing these tasks in a very timely manner.
2x vacancies. One of the roles is working with Young Adults aged 18-25 who have a learning disability and are Care Act eligible. The role will involve:
- Undertaking Care Act Assessments
- Completing support plans
- Completing reviews
- Safeguarding (all statutory work)
- Case management/oversight that is required
Caseload is 20-25.
Flexible working arrangements can be offered but all visits and the majority of meetings need to be in person. Need to be in the office around twice a week.
