Experienced Qualified Social Worker required to work in the Hospital Discharge Urgent Response Team. The role is fast-paced with a focus on the prevention of going into hospital. Cases are held for 5 days before being referred over.

  • 3 month rolling contract
  • £32ph
  • ASAP Start
  • Duty in office only – Monday to Friday 1 week a month
  • Rest of the time can be remote/flexible on working location + visits
  • Based just outside of Tower Hill station – South Norwood
