Westminster City Council is seeking a skilled and motivated Social Worker to join our South Complex Care Team. This role is based in Victoria with a hybrid working arrangement, requiring regular office attendance with the flexibility to work from home as agreed with the manager. You will be responsible for conducting Care Act assessments, developing care plans, performing risk assessments, reviews, placements, Continuing Healthcare (CHC) assessments, Mental Capacity Assessments (MCA), and managing referrals.

Experience using MOSAIC or FrameworkI (FWI) systems is advantageous, and the role requires strong report writing skills and proficiency in MS Word, Outlook, Teams, and other MS Office applications. Additionally, you will lead safeguarding enquiries and work closely with the Safeguarding Operational Team.

Key Responsibilities:
  • Conduct Care Act assessments, care plans, and risk assessments.
  • Perform safeguarding work as a Lead Enquiry Officer.
  • Duty work including occasional duty visits and casework for customers in urgent need.
  • Work closely with a multi-disciplinary team (MDT) and attend regular team meetings.
  • Utilize MOSAIC/FWI systems for care management and reporting.
  • Support individuals in gaining independence and delivering strength-based work.

  • Social Work England registered.
  • Strong knowledge and experience of working within a Local Authority, particularly in complex care.
  • Ability to work independently and as part of a team.
  • Demonstrated experience in safeguarding and providing high-quality casework.
  • Strong communication and collaboration skills.
  • A commitment to providing first-class service to customers and carers, promoting independence within the community.
