We are looking for a social worker. The Locality teamwork is an ongoing care management intervention, and the coordination team leads on annual reviews of support plans, extra care housing support and direct payment work. This service also provides assessments under Care Act 2014 and works with young people transitioning from Children’s services to Adult services, and beyond.

You will be supporting Younger Adults with physical disabilities and sensory impairment and Older People to live as independently as they are able and work with their carers to ensure risks are appropriately managed. You will need to be flexible, committed, and hard working with a willingness to work to quality standards.

You will be working within a multidisciplinary framework and need to have good communication and negotiating skills. You will be working with Partner agencies and will be required to develop close working relationships with them.

The successful candidate will be required to work as a duty worker on a rota basis, facilitate hospital discharges, undertake Safeguarding (s42) enquiries, and undertake Mental Capacity Assessments/ Best Interest decisions.

We are looking to appoint someone who is suitably qualified, innovative, enthusiastic, motivated, and able to work well as a member of the team.

Flexible Working: Social workers are expected to undertake face to face visits to families. All social workers are expected to be in the office 2 days a week.

Expenses: Locums are able to claim back mileage from their place of work to visits (this does not include travel from home to work). The service also has work oyster cards that can be provided.
