We are looking for a motivated individual to provide a comprehensive confidential secretarial and administrative service for the bed management team as our In patient Discharge Coordinator.

You will work across the adult acute inpatients wards in Hertfordshire inpatient housing and accommodation worker in the discharging of service users back into the community.

The role also involves updating of records on Paris (Electronic patient records) and accessing Spike data relevant to post.

This is an extremely busy and varied role, responsible for all aspects of the Service user discharge pathway administration and will involve daily service user contact, discussions with staff, so excellent interpersonal skills are crucial to this post., To assist the Inpatient team to work on barriers for discharge to ensure that Inpatients can be discharged according to their recovery and wellbeing.

To liaise closely with other agencies to ensure that all plans are in place for a planned discharge.

To liaise and work in close co-operation with the Team Leader, other mental health professionals, and voluntary and private sector agencies as required, to help ensure that best value services are delivered to the users.

To have a comprehensive understanding and implementation of the referral pathways for HPFT services and those available from other agencies and providers.

To work closely with the bed management team to support the discharge of patients who are delayed transfers of care.

To be able to respond in a creative, pro-active and flexible way to urgent situations, using local protocols in order to support discharge plans.

To develop plans in conjunction with other service providers that will enable the provision of practical support to service users and their carers in developing and managing dignity and independence., To develop plans in conjunction with other service providers that will enable the provision of practical support to service users and their carers in developing and managing dignity and independence.

To ensure the accurate and appropriate information is available to enable staff and service users to gain access to resources, including benefits and welfare rights.

To maintain accurate records as required by existing procedures, entering appropriate details on the service users’ case notes / electronic patient records as necessary.

To ensure the service user understands and has a clear pathway of care across sector / agency boundaries with key contact points / named individuals.

To positively promote independent living of service users within community.

To develop a rapport with service user’s, based upon attentiveness, sensitivity, understanding, compassion and honesty

To use information systems including information technology and be willing to undertake appropriate training. To maintain a record of the work undertaken and the diversity of the role.

  • NVQ level 3 qualification or mental health certificate level 3 or agreement to undertake such training as soon as resources permit

  • NVQ/Mental Health Certificate Level 2

  • Commitment to Continuing Personal Development, * Good written communication skills

  • Ability to create innovative solutions to help empower service users to provide practical support with daily living activities

  • Ability to work unsupervised in a range of settings

  • Ability to form positive therapeutic relationships with service users and carers

  • Ability to use structured therapeutic skills, such as solution focussed therapy, or a commitment to acquiring such skills

  • Good awareness of mental health system, local services, benefits/employment systems

Desirable criteria

  • SCIP Trained, Some experience of providing mental health services from within a team setting, which may be combined with personal lived experience of mental health difficulties, Ability to be versatile and accessible

  • Ability to prioritise a workload within agreed deadlines
