The post holder will be an expert clinical practitioner who will work in collaboration with the Coronary Heart Disease Specialist Nurse team to deliver a nurse-led, evidence-based Secondary Prevention Service, in accordance with patient need, local guidelines, and the National Service Framework.

Responsibilities

  • Be responsible for coordinating the delivery of the service, service development, and service improvement.
  • Act as an expert practitioner acting as a resource, ensuring all patients, relatives, carers, and staff have ready access to Coronary Heart Disease nursing support and advice.
  • Represent South Tees Hospitals NHS Foundation Trust, Community Division, when dealing with external agencies and have responsibility for developing policy across the Coast-to-Coast region.
  • Undertake other tasks and duties as required which are commensurate with the nature of the post.
  • Show due diligence in the adherence to and perpetuation of STHFT policies and procedures.
  • This job description represents an outline of the responsibilities of the post and may be amended following consultation with the post holder.

Minimum Requirements

  • Access to a vehicle for work purposes to meet the requirements of the role.
  • Deliver holistic, Secondary Prevention Clinics throughout the Redcar and Cleveland area.
  • Provide regular training to members of the primary health care team (e.g., medical students, nurses, GP registrars).
  • Provide specialist professional development and ongoing training both formal and informal, involving presentations at trust educational events (including those external to the Trust).
  • Provide specialist assessment in the field of Coronary Heart Disease for all members of the Primary and Secondary Health Care Team and bring the perspective of a front-line clinician to the development of service strategy and policy.
  • Work with agencies external to Redcar and Cleveland, acting as an ambassador and being instrumental in ensuring the delivery of care across the secondary care and primary care interface is as seamless as possible.
  • Engage in effective clinical reflection on a structured basis and participate in appraisal and personal development planning.
  • Ensure personal and professional update in Coronary Heart Disease and related matters, including national policy changes.
  • Act as experts in the field of Coronary Heart Disease, in Primary Care, by providing specialist packages of care to patients.
  • Improve patients' understanding and management of their condition.
  • Build and maintain effective communication networks with external agencies to ensure seamless pathways of care.
