Working across primary, secondary, and community care, you will provide expert clinical leadership, influence policy, and develop innovative approaches to improve outcomes for older patients. Main duties of the job
As a Consultant Pharmacist in Frailty & Overprescribing, you will lead medicines optimisation for older people across South East London ICB and GSTT, tackling medication-related harm and improving patient outcomes. Key responsibilities include:
- Clinical Leadership: Provide expert guidance on medicines use in frailty, multimorbidity, and polypharmacy, accepting referrals and advising clinicians in primary and secondary care.
- Reducing Overprescribing: Lead system-wide strategies to implement best practices, ensuring safe and appropriate prescribing.
- Service Development: Shape and enhance pharmacy services across the ICS, working collaboratively with PCNs, community pharmacies, local authorities, and social care.
- Education & Training: Mentor and support healthcare professionals, delivering training on frailty, medicines safety, and deprescribing strategies.
- Research & Innovation: Drive quality improvement, conduct research, and contribute to national overprescribing initiatives.
- Policy & Strategy: Influence local and national policies, ensuring alignment with NHS priorities and ICS objectives.
- Collaboration: Work closely with ICS partners, regional and national stakeholders, and patient groups to promote person-centred care.
