We are seeking a dedicated Social Work Assistant to join the Adult Services Review Team. Based at County Hall, you will work closely with Social Workers, Duty Workers, and the Team Manager to support individuals in need of care and assistance. This is an excellent opportunity for someone looking to make a real difference in the lives of vulnerable adults.
Key Responsibilities
- Arrange and monitor appropriate services in line with individual care plans.
- Provide advice and practical assistance to service users.
- Liaise with voluntary and statutory agencies to coordinate support.
- Complete annual care plan reviews and reassessments.
- Participate in team meetings and duty rotas.
- Work in accordance with Council policies and procedures.
- Undertake training and contribute to the development of others.
