Hartlepool Borough Council is seeking an experienced agency social worker to join our Disabilities Team. This is a fantastic opportunity for someone with a passion for supporting individuals with disabilities to make a positive difference in their lives.

Key Responsibilities:

  • Provide high-quality social work support to individuals with disabilities.
  • Assess the needs of service users and create tailored care plans that promote independence and well-being.
  • Work collaboratively with families, carers, and multi-disciplinary teams to ensure the best outcomes for service users.
  • Manage a caseload of complex cases while maintaining high standards of practice.

Requirements:

  • Experience: A minimum of 2 years' experience as a social worker, ideally within a disabilities setting.
  • Qualifications: A recognised social work qualification and HCPC registration.
  • Knowledge: Understanding of relevant legislation and frameworks, including the Care Act and safeguarding protocols.
  • Hybrid Working: Hartlepool Borough Council has adopted a hybrid working model. Flexible working arrangements will be discussed with the manager.
