\\nThe aim of the team is to provide impactful strengths-based working at the front door to prevent, reduce or delay needs from escalating to maximise independence and enable Manchester residents to achieve their best outcomes at the earliest opportunity.\\nThis opportunity gives you experience of being at the forefront of social work, as the service is the first point of contact in Adult Social Care, for adults who are experiencing a range of situations. \\nYou will be the social worker with the mental health specialism within the team delivering the triaging function.\\nUsing your professional knowledge and expertise as a professional resource to deliver strength-based assessments and support for residents alongside being a professional resource for other ASC & Health colleagues. \\nYour aim is to resolve these by providing short-term interventions and completing assessment to achieve person-centred outcomes.
These outcomes can range from signposting and providing information and advice right through to the provision of early help, targeted support services, crisis intervention and adult safeguarding responses when needed.\\nYou will be an experienced mental health social worker with two years of post-qualifying experience with substantial knowledge of ASC and specialist knowledge about mental health difficulties gained by working with residents. \\nYou will be able to ensure that all work is carried out in line with the appropriate legislation, (Care Act 2014, Mental Health Act 1983, Mental Capacity Act 2005, Human Rights Act 1998) guidelines, policies and procedures.\\nThis will require you to work collaboratively with colleagues. \\nYou will use your knowledge of early help and secondary care offers to provide information and signpost people to the most appropriate services.
\\nYou will be working in a fast-paced environment, be resilient to engage in evolving ways of working and deliver innovative and creative solutions. \\nYou will identify risk and explore risk management and mitigation in high risk and complex situations\\nA key element of the role is developing effective working partnerships with a wide range of internal and external agencies to support delivery of the new front door model and an innovative and responsive early help offer.
