Leaving Care Personal Advisor Location: Bradford Position: Full-Time, 3-Month Initial Contract (with potential for extension) Hours: Full-time, including occasional evenings and weekends Are you passionate about helping young people who are in care or transitioning out of care? Your responsibilities will include: Assessment & Monitoring : Supporting the assessment and ongoing reassessment of needs, ensuring that plans remain effective and tailored to each individual. Personal & Emotional Support : Offering practical support to promote physical, emotional, and social well-being, including assisting with domestic tasks and providing transport when necessary.
