To provide care for patients in a holistic manner incorporating physical and psychological care to enable them to live with a cardiac condition.

To work as an independent and autonomous practitioner within a multidisciplinary team, ensuring that patients with Coronary Heart Disease (CHD), those recovering from cardiac surgery, ICD implantation, or diagnosed with Heart Failure, receive the highest standards of care and comprehensive support from diagnosis through every stage of their condition.

Demonstrate excellent clinical assessment skills with a view to optimising patient’s medication.

To provide professional support, clinical expertise and appropriate education and training to all those professionals involved in providing care to patients .

To assist in leading and developing CHD and Heart Failure care to meet the requirements of the NSF quality requirements and NICE/ESC guidelines ensuring a continuous process of evaluation.

Undertake an active role in promoting and carrying out research in conjunction with other health professionals – seeking to have work published.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
