The predicted salary is between 36000 - 60000 £ per year.

We are looking for a two discharge coordinators to complement our existing ICT North and West teams. We are looking for individuals who will be able to provide continuous coordinated support to the safe and timely discharge of patients from the caseload, working collaboratively within the Intermediate Care Team (ICT).The post holder will co-ordinate timely patient referrals to relevant community services and liaise directly both with patients and their relatives as required about discharge arrangements. The post holder will also need to liaise with other key stakeholder’s integral to the discharge process. The post holder will work with colleagues to support discharges over a 7-day period to meet the needs of the patient. This will require some weekend working on a Rota basis over 7 days a week period., Attending Team MDT meetings, with prior preparation to discuss complex cases and longest days on the caseload with senior team.

Completing discharge notifications for onward referrals to other discharge services

Completing all referrals to other services such as community nursing, adult social care, therapy, equipment and voluntary services.

The post holder will be expected to communicate with patients who may have communication difficulties, families and other colleagues.

Liaise with patients and give advice where appropriate regarding their care

Post holder will ensure all patients documentation and pathway is documented electronically and that protocols are followed for data collection purposes.

The post holder is required to input all discharge and admission data to support KPI and monitoring.

The post holder will be involved in the induction of new colleagues and provide support and guidance as necessary.

Support complex discharge home assessments if required

Proactive management for patients with no reason to remain on the caseload for more than fourteen days.

Proactive Management/allocation of the routine band 5 nursing visits to ensure flow through the caseload

Review the package of care waiting list to identify delays and to ensure patients are being discharge in a timely manner to appropriate services.
