Job overview
The 18-25 offer is based within CAMHS in the vibrant borough of Waltham Forest. We offer flexible, needs-based, clinical interventions to young adults aged 18-25 within a variety of community spaces.
We work collaboratively with Specialist CAMHS, adult mental health services, colleges, family hubs, social care and voluntary services in order to ensure we are accessible and inclusive for all young adults we work with.
The team currently consists of mental health nurses, social workers and a mental health/participation clinician who are all committed to working in a co-productive, inclusive and reflective style.
Main duties of the job
- Ensure the smooth transition of the care of young people from CAMHS to adult mental health services.
- Work with YA (18-25 years) who have not previously/recently been known to CAMHS, to support accessing and navigating adult mental health and other community services.
- Support the wider partnership work to develop and improve pathways to support the transition and delivery of an integrated offer across agencies (including education, social care, and the voluntary sector) as indicated in the LTP requirements for 18-25-year-olds.
- Work across the age range and facilitate partnership working to support these young people develop daily life skills, access employment opportunities, social networks etc.

Probationary Period
This post will be subject to a probationary period. NELFT places great importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and undertake a comprehensive induction of up to two weeks, including mandatory training, systems training and the allocation of equipment.
