Finegreen are currently supporting a large healthcare organisation in the appointment of a senior clinical professional, to provide leadership and day to day management of the community palliative care team to ensure the service delivers high quality patient care across the community.

Key Responsibilities:

  1. Overall accountability for the Integrated Community Palliative Care Team, providing operational and strategic business planning and leadership of the services, dealing with all practice issues, and ensuring deadlines are met.
  2. Managing key performance indicators (KPIs) and service measures ensuring high-quality patient care is delivered.
  3. Motivating and managing the community palliative care team including nurses, healthcare assistants, clinic and outreach staff including Allied Health Professionals (AHPs) and community engagement, leading on the management of the shared community caseload.
  4. Continuously building and improving relationships, communication, and teamwork with health service providers across the hospice and externally, including GPs and hospitals, raising awareness of the organisation by representing Community Palliative Care at appropriate meetings and promoting its function both internally and externally.
  5. Ensuring regulatory compliance with Care Quality Commission (CQC) standards and key lines of enquiry; implementing and monitoring action plans as required for internal CQC audits and leadership walkabouts, complex complaints, and incidents.
  6. Regularly reviewing and communicating risks with staff and identifying areas where improvement is required; contributing to Serious Untoward Incidents or Duty of Candour incident investigations and investigating complex complaints and concerns related to the community or other services as required.

Ideal Candidate:

  1. Must hold current registration to the NMC as a Nurse; advantageous if you have a post grad in the area of community nursing or palliative care.
  2. Demonstrated track record of managing a multidisciplinary team within a community setting.
  3. Understanding of life-limiting illnesses and pathways related within adult-based care.
