Thornbury Medical Centre are seeking a part-time or full time experienced and enthusiastic Practice Nurse. GP Practice experience is essential.
The post holder is responsible for the delivery of practice nursing services and care to the practice population. Supported by GPs, Practice Manager and the Lead Practice Nurse, you will deliver care within your scope, focus on supporting patients to be healthy, monitoring long-term conditions, health prevention and screening activities.
You will work collaboratively with the whole team to meet the needs of patients and improve outcomes, supporting the delivery of policy and procedures.
The post holder will also strive to maximise the practices achievement of both Local and National Quality and Performance objectives., Experienced in Primary Care, the applicant can expect to be involved in Chronic Disease Management (Diabetes, CVD, Respiratory Disease and of course Long Covid etc), Cervical Screening other Quality and Improvement Goals e.g. Learning Disability Health Checks and Mental Health Physical Health Checks, Vaccinations and Immunisations (Adult and Child), Cervical Screening, Spirometry, ECGs, Contraception and Sexual health Advice, Travel Vaccination and Advice, Ear Irrigation, Spirometry, Wound Care and Venepuncture., On occasion it may be necessary to undertake home visits (e.g. flu vaccination and long term disease monitoring).
Good Knowledge of Clinical Governance and Infection Prevention Control principles are essential. Good standard of written English and computer literacy are required., Proficient in providing compassionate patient care.
