You will be required at the surgery premises and elsewhere to provide medical care including related administration as follows: Working booked surgeries – 14 booked patients per surgery, to include both reactive and proactive medical care. Visiting housebound patients – Most visits are undertaken by the Urgent Care Practitioners, there may be occasions when you are asked to undertake a visit or you choose to as part of the care of a patient you know well. On call duties up to a total of 2 half days a week for a 6 session GP. Extended hours duties 1 hour each week.
Associated paperwork. Record keeping using the EMIS Web system – The successful candidate is required to keep proper records of attendance, visits by and to any patients and all such other records as are required by NHS legislation or are reasonably required by the Practice. To work with the partners, the patient and their carers to ensure that patients at risk of hospital admission have a relevant and up-to-date care plan.
