  • Working with adults who are diagnosed with mental health conditions in secondary mental health care, including: dementia, psychosis, anxiety and depression.

  • Work as part of a multi-disciplinary Community Mental Health Team alongside health and AHP colleagues.

  • Knowledge and experience of implementing the Care Act and Mental Capacity Act is essential.

  • Work directly with service users and their carers using a range of knowledge, skills and interventions as required, developing an individualised approach to the practical and functional needs of the service users.

  • Recognise various mental health problems and undertake or participate in holistic needs based assessments, produce a care plan, implement and review the plan, including the management of risk.
