Working with adults who are diagnosed with mental health conditions in secondary mental health care, including: dementia, psychosis, anxiety and depression.

Work as part of a multi-disciplinary Community Mental Health Team alongside health and AHP colleagues.

Knowledge and experience of implementing the Care Act and Mental Capacity Act is essential.

Work directly with service users and their carers using a range of knowledge, skills and interventions as required, developing an individualised approach to the practical and functional needs of the service users.

Recognise various mental health problems and undertake or participate in holistic needs based assessments, produce a care plan, implement and review the plan, including the management of risk.
