The Clinical Home Manager is responsible for the management and coordination of clinical care services for patients receiving care in their homes. This includes overseeing clinical staff, ensuring compliance with healthcare regulations, providing direct patient care as needed, and liaising with other healthcare professionals to optimize patient outcomes. The Clinical Home Manager plays a critical role in improving the quality of life for patients and their families. Key Responsibilities: * Oversee the day-to-day clinical operations of home healthcare services. * Supervise and manage a team of healthcare professionals, including nurses, therapists, and support staff. * Develop, implement, and review care plans tailored to individual patient needs. * Conduct regular patient assessments and ensure the delivery of safe, effective care. * Ensure all services comply with healthcare regulations and industry standards. * Monitor patient progress, adjusting care plans as necessary and addressing concerns or issues. * Provide clinical leadership, mentoring, and support to the home care team. * Collaborate with physicians, family members, and other healthcare professionals to ensure holistic patient care. * Address any patient or family complaints or concerns, ensuring appropriate resolution. * Maintain accurate and up-to-date patient records and documentation in compliance with healthcare laws. * Conduct regular training sessions for staff to ensure the highest level of care. * Manage clinical budgets and resources effectively, optimizing care delivery while maintaining cost efficiency. * Stay current on industry trends, new treatments, and best practices in home healthcare. Qualifications: * Registered Nurse (RGN) or licensed healthcare professional with clinical experience. * Minimum of 5 years of experience in healthcare, with at least 2 years in a clinical leadership or managerial role. * Excellent communication, leadership, and interpersonal skills. * Strong problem-solving abilities and the ability to work under pressure. * Ability to work independently and as part of a multidisciplinary team. * CPR/First Aid certification (or equivalent) preferred
