Social Workers Triage and Response Team

£32 per hour Umbrella company

Bristol City Council are looking to recruit 3 social workers to join their Triage and Response Team, which is integrated within the discharge teams at Southmead Hospital and Bristol Royal Infirmary Hospital. Initial contracts offered until mid March 2025. Full time hours required. There is a requirement for social workers to be in the office everyday unless there are reasonable adjustments that BCC will need to consider.

To be considered for this role you will have a passion for enabling service users to leave the hospital in a timely manner with appropriate support in place. Social Workers will work in coordination with health colleagues in discharge teams to provide appropriate, strengths-based, and timely support.

Responsibilities include: Completing Care Act assessments, mental capacity assessments, and other assessments in line with legislation, developing comprehensive discharge plans, and collaborating with medical staff to ensure seamless patient care transitions.

To apply for this role, you will need to hold –

  • A minimum of 6 months experience of working in the UK as a registered social worker
  • A relevant Social Work Qualification
  • Registration with Social Work England
  • Enhanced DBS, registered on the update service or be happy to apply for a DBS
  • 5 Years of referencing (specific for Bristol CC agency appointments)
  • Driving license with access to a car
