Interim Associate Director Urgent Care, Discharge and Care Homes

To deliver high quality, integrated out of hospital discharge pathways across acute, intermediate and community health and social care provision as well as take the strategic lead for care homes. Accountable for Delayed Transfer of Care (DToC) the post holder will engage with national and regional policy makers and assurance processes, as well as working with colleagues from across the STP to lead a coherent strategic approach across the area.
